Any student in Harrison County with a special health care need or taking school administered medication will require an individualized health care plan.  This information may be either mailed to the parent from the school nurse or can be printed from the following links.  Please click on the appropriate diagnosis for your student and print off all forms to be hand completed at home and returned to school nurse.
